To ensure timely withdrawals, please complete the Credit Card Authorization Form for all the cards you have used to deposit in the last 6 months. Please complete every field and send it along with legible copies of:

1. Front and back of all Credit Cards used. (Make sure the back panel of the card is signed).
2. Government-issued personal identification (driver's license or passport).
3. A recent utility bill (phone bill or electricity bill).
4. Bank statement for any of the cards.
